Fill in X. It may be something that we need to figure out. Taking a swing at this. Handling the ability of PCs to breathe underwater (ie drowning) Con x 5 for 1st round Con x 4 for 2nd Con x 3 for 3rd.....so on. Debate for this? This what you're looking for?

Will there be a significant sanity gain/loss at the end of every adventure/transport to a new realm/universe? (Similar to regular HC, or will the "Oh god, I am now in a universe where Justin Beiber died at birth, and I am going to stay here now." effect take over?)
Other thoughts
-Handling the monkey ability to use highly technological weapons....will this be "give me learning after your basic induction training into the Lazer Guards!" or "Guns at -60"?

Those two are simple and I can answer here:
Depends on what the players did. Wide gaumet of stuff there between 'showing up as a quick tourist' to 'saving the day'. There won't be any automatic sanity gains - the players have to have accomplished what the GM considers something. The player may have set the goal but in the end, it's all about what the GM feels like with san rewards. As per normal.
New skills will start out at learningx1.
