Handling the ability of PCs to breathe underwater (ie drowning)
Con x 5 for 1st round Con x 4 for 2nd Con x 3 for 3rd.....so on.

I'm thinking it takes a couple minutes (3 according to books - much much less for regular people) to start drowning IF someone has gotten a good breath of air before they went underwater.
If they haven't, it's probably much quicker.
Since the combat round is a couple seconds, I'm thinking having them drown real fast isn't really a good thing.
My initial guess would be (assuming they didn't get a big breath first and now they're in water. And in combat) to have a con/health/endurance roll at full. If they make it, assuming the combat is under five or ten rounds I wouldn't worry about it.
If they fail it, give them a d6 (??) stun damage and then carry out combat.
If the PC's are trying to do something different like 'An under water swim that is so long it requirres a con/health/endurance roll at -30 or they will take 2d6 stun during this huge long swim. If they survive that, they make it to the place.' I would just have that written up as such. If they go unconscious and their buddy decides to grab them and continue to swim (now at -30% carrying your unconscious ass along) etc.

The fast drowning was like "How long was he under water? Less than 20 seconds. And he drown?"

I want to say you started at like Con x 10 in the old rules... I think you're right. But even if you survived 20 rounds - somehow - that's still less than a minute. If you're a smoker in horrible shape (like me) that might kill you but I'm thinking I'd have at least even odds - better if someone was there with CPR and stuff of surviving 40 seconds without air.

Well, failing the roll should mean you lose consciousness, not you die immediately. Also, you shouldn't have to start rolling until you've been under for at least a minute or so. Maybe conx3 rounds? I think the rule of thumb is that pretty much anyone can go without air for 2 minutes without risking permanent damage. en.wikipedia.org/wiki/Drowning#PrognosisLooks like you can be rescued and have a chance at survival up to 25 minutes underwater.
