Every night the PC's sleep, they can do a willpower roll.
Fumble: You stay wounded this week - try again seven days from now. Also, you are back at 'residual self image'. Any sort of implants, etc - gone. Fail: You stay wounded - try again tomorrow night. x5: Heal 1 real HP. x4: Heal 2 real HP. x3: Heal 3 real HP. x2: Heal 4 real HP. x1: Heal 5 real HP. Crit: You're fine. Total heal.

FA/M can give HP back right now as opposed to after a good night of sleep. Though first aid is still useful for the PC's (but to a more limited way) I'd be very surprised if any PC ever stayed in a hospital.

How I'd planned to do it turns out not to be how I have been doing it. Rather than trying to change my brain (harder) change the rules as this way has been working.
Every night the PC's sleep, the PC can call for a willpower roll. (If they do not, they stay wounded but need not roll - their choice).
Fumble: You stay wounded this week - try again seven days from now. Also, you are back at 'residual self image'. Any sort of implants, plastic surgery, etc - gone. Fail: You stay wounded - try again tomorrow night. x5: Heal 1 real HP. x4: Heal 2 real HP. x3: Heal 3 real HP. x2: Heal 4 real HP. x1 OR CRIT: You're fine. Total heal. Also, you are back at 'residual self image'. Any sort of implants, plastic surgery, etc - gone.
Note - the changes to the crit aren't actually changes as I had this in the notes for the campaign:
Basically, four things can return a character to ‘residual self image’ IE how they started out:
1. A critical or x1 willpower roll when healing/sleeping. Note - this is whether they wanted it or not. They can spend a HERO Point to attempt NOT to get a critical if they want. Example: Someone is really beat up and wants to heal. They roll a critical on a willpower roll while healing. If they keep this roll, they will be fully healed however the plastic surgery they had will revert back to their old face/body and any cyberware or new form they had is likewise gone.
2. A fumbled willpower roll when healing/sleeping may get rid of things like plastic surgery, cyberware but keep massive wounds. Certain unhelpful items (such as a brain bomb) may end up sticking around because the universe does not love you.
For those interested in what 3 and 4 are:
3. Dying. If a character respawns, they are automatically set to residual self image.
4. One of the special (HERO Point overflow) powers can cause the character using it to revert to their ‘residual self image’.
Cannot be counter spelled etc.
