RESIDUAL SELF IMAGE
Residual self image is totally healed up and back to exactly the way they started the campaign out.
Basically, four things can return a character to ‘residual self image’ IE how they started out:
1. A critical willpower roll when healing/sleeping. Note - this is whether they wanted it or not. They can spend a HERO Point to attempt NOT to get a critical if they want. Example: Someone is really beat up and wants to heal. They roll a critical on a willpower roll while healing. If they keep this roll, they will be fully healed however the plastic surgery they had will revert back to their old face/body and any cyberware or new form they had is likewise gone.
2. A fumbled willpower roll when healing/sleeping may get rid of things like plastic surgery, cyberware but keep massive wounds. Certain unhelpful items (such as a brain bomb) may end up sticking around because the universe does not love you.
3. Dying. If a character respawns, they are automatically set to residual self image.
4. One of the special (HERO Point overflow) powers can cause the character using it to revert to their ‘residual self image’.

If your residual self image has changed due to an insanity gained (eg believing you are someone else etc), will your physical form change to be closer to that of the insanity?

No - the residual self image is how you first came in, regardless of how crazy you've since gotten.
Also, the residual self image if gained from death can clear other things that the players don't want cleared (alliances and such) but it may be for the best.

Note - the RSI also would clear things like 'weird diseases', becoming a werewolf, etc.
