Not just for killing people or things like people. It can be for a wide range of things. Generally, if something looks scary and is attacking you, you should be fine.
But there are other things you can pick this up for that are not even human shaped.

We should at least limit it to sapient beings, I would think.
If you are wanting to hand it out for more routine stuff, you should remove the card that punishes players for having points toward CBK and lower the maximum.

My rule of thumb was always "sapient beings that don't cause san loss just to see".
If you're going to bend/break that rule, I would try and tie it to the zone rules. E.g., Sleestak in the Sleestak States of America zone(Do we have a consensus name for this zone?) count for CBK even though they still cause san loss.

Ghouls are sentient, cause sanity loss.
Some beings cause CBK, some don't. Some cause sanity loss, some don't.
Example: Killing sleestak in Voras gives sanity loss. Killing them in dino land (as the PC's have been calling it), no sanity loss.
While I don't need to 'tie it to any special zone rules', if you want to do that to keep track of what you think is going on, you are welcome to do so.
